Is Cast Iron Safe to Cook With?

Yes — bare cast iron is one of the safest cooking surfaces there is. It has no chemical coating, no PFAS or nonstick chemicals, nothing but iron and the polymerized-oil seasoning you build on it. The main thing it adds to food is a little dietary iron, which for most people is a benefit, not a risk. The one group who should be cautious is people with an iron-overload condition.
Cast iron's safety is actually one of its selling points, so it is worth laying out plainly what a cast iron pan does and does not put in your food.
No coatings, no "forever chemicals"
Bare cast iron has no manufactured nonstick coating at all. The slick surface comes from seasoning — cooking oil that has been heated until it polymerizes into a hard, inert, food-safe layer, chemically similar to the coating on any well-used pan. There is no PTFE, no PFOA, and none of the per- and polyfluoroalkyl substances (PFAS) associated with some nonstick cookware. That absence is exactly why many people switch to cast iron in the first place.
The iron it adds is usually a benefit
Cooking in bare cast iron leaches a small amount of iron into food, more so with acidic dishes like tomato sauce and with longer cooking. For most people this is a good thing: iron is an essential nutrient, and many diets are short on it, so a cast iron pan is a mild, natural way to add a little. The faint metallic taste a long tomato simmer can pick up in a bare pan is simply that iron; it is harmless, and using an enameled pot for long acidic braises avoids it if you do not want it.
The one real caveat: iron overload
The extra dietary iron that helps most people is a reason for caution for a small group: those with iron overload conditions such as hereditary hemochromatosis, who absorb and store too much iron. If you have such a condition, it is worth moderating how much you cook acidic food in bare cast iron, or favoring enameled cookware, and following your doctor's guidance. For everyone else, the added iron is negligible-to-helpful, not harmful.
What about enameled cast iron?
Enameled cast iron is coated in vitreous enamel — essentially fused glass — which is inert and does not leach iron or react with food. Reputable brands make their enamel lead- and cadmium-free and test to food-safety standards. The sensible caution is with very cheap, unbranded imported enamelware, where quality control is less certain; buying an established brand removes the worry.
Is a rusty or old pan dangerous?
A little surface rust on a cast iron pan is not toxic — you would not want to eat flakes of it, but it is just iron oxide, and the fix is simply to scrub it off and re-season.
